Al ricevimento del modulo, verrete inseriti in una lista d'attesa e poi contattati per un appuntamento. Al primo appuntamento portare una copia di un documento d'identità valido. Grazie.

• if this is not possible, pick up the form at the desk of the Gynaecology-Polyambulatory (upper ground floor)
