• Wenn dies nicht möglich ist, das Formular am Schalter der Gynäkologie-Poliambulatorien abholen (Hochparterre)

Nach Erhalt des ausgefüllten Formulars für das Erstgespräch werden Sie auf eine Warteliste gesetzt. Sie werden dann für den ersten Termin kontaktiert.  Beim ersten Termin bitten wir Sie, einen gültigen Personalausweis mitzunehmen. Vielen Dank.

For a first visit

• download the attachment
• fill it in, writing in legible capital letters
• send it by e-mail gyn-amb.me@sabes.it
• If this is not possible, pick up the form at the desk of the Gynaecology-Polyambulatory (upper ground floor)

Upon receipt of the form, you will be placed on a waiting list and then contacted by for an appointment. At your first appointment, please bring a copy of valid ID card. Thank you.
